How to Become a New Patient
We are here to support you on your journey to better mental health. Becoming a new patient at our clinic is simple, and we’re here to guide you every step of the way. Here’s how to get started:
1. Schedule a Consultation
To begin, reach out to our office by phone or through our online contact form. One of our team members will help you schedule an initial consultation to understand your needs and match you with the right provider.
2. Complete the Intake Forms
After scheduling your appointment, you’ll receive a link to the client portal where you can easily complete intake forms that provides us with important information about your medical history, mental health background, and treatment goals. Filling this out beforehand ensures that we can provide you with the best possible care. It is important for you to complete the forms 72 hours prior to your first scheduled appointment or your appointment may be cancelled or rescheduled.
3. Meet with Your Provider
During your first visit, you will meet with one of our mental health professionals for a comprehensive assessment. This will include discussing your concerns, goals, and any specific treatment preferences. Together, you and your provider will develop a personalized treatment plan tailored to your needs.
4. Begin Your Treatment
After your initial assessment, we will start your treatment plan, whether it involves therapy, medication, or other supportive services. We will ensure that you feel comfortable and supported throughout your care journey.
5. Ongoing Support and Care
As a new patient, you will have continuous support from our team. We will regularly check in on your progress and make adjustments to your treatment plan as needed to ensure your well-being.
6. Follow-up Appointments

Patient Responsibilities

As a patient, your responsibilities include:
1. Attending Appointments
Consistent attendance at scheduled appointments is key to your progress. If you are unable to attend, please notify us at least 24 hours in advance so we can reschedule.
2. Honesty and Openness:
To provide the best care, it is important that you share accurate information about your mental health, history, symptoms, and treatment goals. Open and honest communication will help us tailor your treatment plan to meet your specific needs.
3. Participation in Treatment:
Actively engaging in your treatment, whether through therapy, medication management, or other recommended services, is essential for achieving positive outcomes. If you have concerns or questions about your treatment plan, please discuss them with your provider.
4. Respecting Practice Policies and Procedures
Please follow the practice’s policies, including confidentiality agreements, payment processes, and behavior expectations. Respecting these help maintain a safe and supportive environment for everyone.
5. Keeping Track of Progress:
We encourage you to monitor your progress and take note of any changes in your symptoms. This will help guide your treatment and provide valuable insight to your care team
6. Communication and Feedback:
We will primarily communicate with you through our client portal. If this method is not suitable for you, please let us know your preferred communication method.

7. Respecting Others:
