Forms/Letters: Disability, Family, and Medical Leave

Letters for disability, work/school accommodations, and family medical leave are only provided to established patients who have been under our care for at least 6 months and are actively participating in both psychotherapy and medication management. Due to the time and information required to accurately assess your prognosis, we are unable to complete forms for new patients.

Letters and forms for work or school-related purposes are typically completed during your scheduled appointment to ensure accuracy. Please allow 7-10 business days for the completion and delivery of your letter via the patient portal if not completed during appointments.

Forms and letters requested outside of appointments will incur a fee, which varies depending on the type and number of pages.

Controlled Substances Policy

At our practice, patient safety and responsible prescribing are our top priorities. In line with regulations set by the Drug Enforcement Administration (DEA), we adhere strictly to guidelines when prescribing controlled substances. These medications, including benzodiazepines (e.g., Xanax, Clonazepam, Ativan), hypnotics (e.g., Lunesta), and stimulants (e.g., Concerta, Adderall), carry a high potential for abuse and adverse effects, and are only prescribed when clinically necessary as part of your treatment plan.

Before prescribing any controlled substance, patients will be required to undergo a urine drug screen. This screening will also be conducted periodically at the provider’s discretion to ensure responsible use and to monitor for any signs of substance misuse or abuse. If substances such as marijuana, alcohol, or excessive caffeine are detected, it may influence the decision to prescribe controlled medications.

Our commitment to your well-being includes minimizing the risks associated with these powerful medications. We are dedicated to working collaboratively with you to explore alternative treatment options that align with your goals and needs. Your health and safety remain our primary concern, and we encourage open communication regarding any questions or concerns you may have about this policy. Please feel free to discuss them with your provider.

Cancellation and Rescheduling

We kindly ask for at least 24 hours’ notice if you need to cancel or reschedule your appointment. This allows us to offer the time slot to another patient who may need it.

Late Cancellations/No Shows

Appointments canceled with less than 24 hours’ notice or missed without prior notification may incur a fee of $75, in accordance with our practice policy.

Payment Policy Notice

In compliance with healthcare laws and regulations, we require payment of copays and any outstanding balances before the start of your visit. This policy helps us maintain the ability to provide high-quality care to all of our patients.

Copayments and Outstanding Balances

Copays will be collected at the time of your visit. Failure to pay your copay may result in the cancellation of your appointment, potentially delaying medication refills. Any outstanding balances must be paid before your next appointment. Unresolved balances may also affect your ability to receive prescriptions for your medications.

Timely payment of copays, co-insurance, deductibles, and outstanding balances helps us comply with legal requirements and ensures the financial health of our practice. This allows us to continue focusing on providing the best care possible for all of our patients.
